As far as I know the only difference between my camera and your camera is the placement of the mics - but who know what changes might have been made at that time to the components and layout of other circuitry when they retooled the production line for that change.
Basically I would say that firmware 1.21r1 was just to intorduce the RAW capabilities ans 1.21r2 was to fix a bug in the RAW photos. I don't think anything else really changed unless it was accidentally affected by that change. Perhaps we expect something else to change and, based on past experiences, are scrutinising everything and now noticing things we didn't notice before or, expecting things to have changed for the worst, notice flaws in areas we never noticed them before.
That makes a lot of sense. After a while, all of the old oddities, bugs, whatever, become accepted/endured. Bring out a rev. that's labeled new and we're all over whatever quirks may have been there since 1.15.
1.21r2 is definitely borken. Every attempt to shoot 1080@60 freezes the camera. The time it takes to fail varies, but fail it does. Borken.
1.21r1 failed today. In this case the camera didn't lock up. About 10 seconds after starting to video 1080@60 the video (shot in a car) froze (no car motion, no change in TOD) and blocks of color (typical video "noise") accumulated across the bottom edge of the screen. 20 seconds later the image cleared and caught up with the car's location. Setting the mic to off doesn't turn off the internal mic.

Nope couldn't replicate it with those settings either. I'm assuming the following settings were just typos - otherwise you have different values than me for them TV Mode auto Light Frequency off
While the timing isn't regular (quits exactly 32.5 seconds after starting or something like that), that the camera fails is certain. The glitch I saw this morning was a new one: video trash at the bottom of the screen. But invariably something goes wrong.
